Take a look at some of your cancelled checks in your subsequent bank statement. Most of them with the sole purpose include a couple lines on their endorsement stamps. You could err on the out of danger side and include your company address, but even that can be argued.
I don't think you involve that much information unless you plan on regularly losing checks on the street before you are competent to deposit them. Or, maybe if you are running a multi-million dollar corporation... surrounded by which case I doubt you would be doing this yourself!
