File a complaint with the insurance department of your state.
All insurance companies (health, auto, homeowners and I would expect travel) are regulated by the insurance department of each state.
Contact my insurance broker - after all that is what they are there for.
Oh - you bought direct? That means you must be an expert in insurance and understand what you should do - so why are you asking?
Cynical comments over.
1) You should contact the insurance company to register a complaint - full details are in the insurance policy wording or policy summary/ key facts document which I asume you have. Explain the nature of the compaint and why you feel that you are not being treated fairly.
2) The insurance company have to acknowledge the complaint within a week, provide an initial answer within 4 and a final answer within 8.
3) If their answer and offer of redress is unacceptable then and only then can you contact the Financial Ombudsman Service (details will be included in the insurer's final reply)
First, ask them about their appeals process. In most jurisdictions, Insurance Companies are required to have an internal appeals process. If it was a legitimate mistake, they will fix it and own up to their mistake.
Next, I would go to your local Better Business Bureau. Most insurance companies are most concerned about saving face and keeping a good reputation.
Last, I would go to your government (your local representative or member of parlaiment) and find out if there's an Insurance Ombuds office that you can make a complaint with. Most jurisdictions will require insurance companies to belong to this process.

Rich race have rich people's challenge and can be very complex. However, adjectives the planning for rich people is an "clear book" process that can last a lifetime. True, they may enjoy lots of money to pay for medical bills and estate duty, but is that cost successful? There are courses available in Estate planning that you can transport to learn more.
Health Insurance?
Here is what I support Universal Health Care. I am a single Mom. I actually produce amazing money compared to most. It is not a lot. But resembling 2000 a month. More than enough. I enjoy a child. I choose not to find a job that have healthcare for my kid. I want the government to confer it to me. I also do not pay taxes. I carry paid lower than the table. I basically work similar to an illegal. But but I want those who do pay to salary for my healthcare.
