well... Some services, per medicare's guidelines, must be billed using that hcpcs code. So, simply billing that code doesn't substantiate fraud. However, if the actual service rendered IS covered by medicare, A9270 shouldn't be billed at adjectives. Its customary to have a description of services within the free-form text of the claim form. The code billed to subsidiary should be the exact code medicare denied, with a description or the valid code. (or at a minimum, should Crosswalk to a valid, comparable code.) Either passageway, secondary shouldn't pay cheque based on medicare's denial short a description of service.
So, the answer is... it depends on the actual service rendered.
Sounds a little shady to me. There are definite procedures that aren't covered - the doctor's office have to just suck it up. A call in is a visit and to call for it anything else is fraud and it's a problem. Calling an earache an infection is one thing, but making something up is totally different.
