I want to create the policy for a commercial company, where do I get the information from?
Answers: You've given us nothing to go on. Are you an underwriter? A broker? A risk manager? A business owner? What type of policy? What type of business is this? What type of information are you looking for?
From the underwriters, with what they want to cover; from your attorneys, to include the legal terminology to limit and include exactly WHAT you want to include; and from your state insurance department, to be sure that the policy form complies with state coverage requirements.
If you have no idea where to begin, and you haven't written a policy before, you are NOT equipped to write one now.
You still haven't told us what type of policy. Answers to your previous question posted earlier gave insight into what we would need to answer you. If you don't kow where to look and can't even clarify the question enough for us to help you, then please don't go messing around with something you obviously know nothing about. The risk to you and the company is too great!
Health insurance providers contained by the state of ohio?
i am trying to get condition insurance quotes from as many companies as i can contained by the state of ohio. so far i have come up with:anthem

combined healthcare
humana

Answers: Assurant, Medicare Mutual, and American Community would be a couple of other condition insurance companies dong business in Ohio. Currently, UHC seem to have some of the better rates available.

Medical Mutual is one of the largest robustness insurers in Ohio. Headquartered within Cleveland with a few regional office throughout the state.
They even have facade to face customer service, if you're fundamental (or want to travel to) one of the customer service centers.
All of their customer service call centers are located within the state of Ohio - no outsourcing call center work somewhere else. Because the customer service telephone call centers are all in fact located in Ohio, they also adopt walk within customers.
(As a side note, Medical Mutual is one of the option offered to state employees and retirees.)

Answers: Only if it's reconstructive, like after an accident.
