I requirement a copy of hcfa 1500?

My health insurance carter is wanting my physical therapy claims submitted on something call a hcfa 1500. My PT provider is out of network and say he doesn't do forms. It's been a huge ordeal and insurance company won't budge on reimbursing me until it's sent to them how they want it. So I've found out from research what adjectives I needed, and PT provider gave me a diagnosis code, cpt codes and their import tax id#. So I am trying to figure out where on earth to get one of these forms, and unsurprisingly insurance won't provide me with one. I found some online places, but they want $30 to buy a box of 500! I solely need 2 for crying distinctly! I asked my regular physician if they could just copy me one stale and they said no, they don't do that. HELP! Short of that, it seems that your PT must be contained by network for someone and must do forms or he wouldn't acquire paid by any providers, in-network or not. I would ask again. It's not a complicated point. Offer him $30 to complete the forms for you!

Please note that the current autograph for this form is the "CMS 1500" form, since the governmental agency changed its name from the Health Care Financing Administration (HCFA) to the Centers for Medicare & Medicaid Services (CMS). Out of way, people still refer to the form as a "HCFA 1500," though.

Why should an insurance company horror the pregnancy within India?Is it too dodgy ?

In Ahmedabad,Realiance life insurance company have rejected a woman's application for life insurance on the source she is pregnant more than five months.
She is working in a hospital and doing her medical check up regularly and till presently no other medical problem.
All other insurance companies are also keeping this policy?

Answers: It is because insurance companies find mortality rate of pregnant women in India is too soaring (even higher than Srilanka). Why should they steal risk unnecessarily?
Life is uncertain. On the spring your life risk is covered. Pregnancy is business of two lives.
The mortality rate during the pregnancy is more when one is single.

since considering these reasons No Life Insurance Company cover ones time during pregnancy.

Does workers comp within the state of ga.enjoy set time borders between offer?

sence being hurt surrounded by may 06,surgery in april 07 for a broken spinal column 2 rods and 4 screws,on the chore injury,workers comp. here in the state of georgia,W/C have made one low offfer, is there time margins between offers or can they drag it out and force you to settle for what ever they want.or do you try and hang about them out and hope you don't loose all you own befor you settle. if that's the case how can this be endorsed ?


Answers: There's no time limit between offer.

You don't get twinge and suffering, so when you settle, it's future medical bills and lost wages. Because of that, they usually keep on a while, to let you do the rehab and surgery and analysis, to figure out how much of a irremediable disability you'll be left next to - and a guestimate of what your future medical bills will be.

I wouldn't be too rapid to settle . . . likely it's going to disappear you out of pocket in the long run, if you're for always disabled.
