I need a form for the insurance company that say Myself and my husband have custody of our grandson. So he can be insured thru our policy. Any backing?
Answers: There is no insurance company form.
You need to show the court documents showing that you enjoy legal custody.
While I be doing dependent audits, we had someone try to turn surrounded by a notarized paper signed by the biological mother and the grandparents stating that the grandparents have custody. It wasn't acceptable - with the sole purpose legal court documents prove custody.
If you don't hold legal custody granted by a court already, consequently you need to obtain a lawyer and procure the papers drawn up in court.
When you be awarded custody of your Grandson, you have recieved a daily from the courts that gave you custody. If you do not enjoy that, you can go down to the court house, and ask the clerk of courts for a copy of the custody papers. Sometimes here is a fee, but you should other have these papers handy within case other situations take place to show proof that you are the custodial care taker of your Grandchild. I hope this help! Insurance companies don't grant custody. You'll never take this form, from an insurance company.
You need to obtain it from a JUDGE. In COURT.
