Not counting phone numbers here should only be two numbers on your card - one is the group number, and the other is the MEMBER number, sometimes call the ID number. It used to always be your social shelter number, but that's being phased out.
should be on the front of the card - your identify and subscriber ID and group ID are other names..... oh also - here should be a toll free number on the back of the card that you can appointment. The hospital or doctor might not know... but if you've been to them previously, they could probably help you - since they probably hold a copy of your card.
