The insurance representative wants me to sign a "Proof of Loss" and a "Trust Agreement".
I do NOT get the impression comfortable signing this document. He also will NOT release a copy of the adjuster's report until I sign this.
Do I have to sign this? What are my rights?

These are all contract lingo, spelled out in your policy. You agreed to it, even if you didn't read the policy. If you don't sign, they won't reimburse - and they'll be absolutely right to not pay cheque, as you, the insured, are not cooperating, and don't want to follow the terms of the policy.
you call for to sign it or you wont get a settlement. why dont you want to sign it? its standard and no big promise unless YOU have a rationale we dont know about If the insurance company recover the deductable they have to settlement that to you they can not get salaried both ways.
