Answers:   Insurance companies consider a pre-existing condition as a condition that you've received medical treatment, advice or consultation, or that would incentive a "prudent person" to seek medical treatment, suggestion, or consultation.

PRE-EXISTING - Anything you've had "prior" to you getting coverage. The severity is of no consequence to whether it's pre-existing. HOWEVER, if a policy states they "cover pre-existing conditions" please work out this is ONLY if the don't deny you coverage because of the severity of the conidtion OR rider off (which resources not pay for) anything related to that condition.

Also, if they state pre-existing condtions are covered AND they don't ask any vigour questions on the application (or embezzle one over the phone) you're likely buying a discount card which isn't insurance.

If you own been diagnosed near a serious illness such as cancer, and you are trying to return with health insurance, you will probably be denied since the insurance company will wages out more for your medical care than you will salary to them in premiums. They are a business looking to form a profit, unfortunately, and they don't see plentiful dollar signs in a character who is already sick. Any illness or condition that will cost the insurance company LOTS of money, lol. Yes that would include mental condition. IN FACT, you need to look at your summary of benefits to check and see if you hold any mental illness benefits to open with!! Some form insurance companies wrote provide that benefit at all because it is a big ticket item.

I can't find a legitimate definition, so I think that the carrier are allowed to define a pre-existing condition. Below is one companies definition.

Any medical condition that you have before taking out the insurance and which you still own. Many health insurers will not cover pre-existing conditions, though some will impart partial coverage in correct circumstances. Shop wisely. Any condition that was diagnosed, or you have reasonable fluency of, before you took out insurance. I guess this would include any mental condition.
