Answers: Why would you receive an employee evaluation form?
Is the person an employee of yours?
If it is your responsibility to fill out forms to give to your bosses answer it honestly.
If this has nothing to do with your company or employees, throw it out.
If you are evaluating yourself, you need to be objective, honest, and confident. Focus on how you benefit the firm, your accomplishments over the period being reviewed, your strengths and skills that have improved. If you have obvious weaknesses, such as a lack of knowledge or skill in an area, address how you are proactively working to improve your skill set. Sell yourself as a team player, valuable to the firm, growing in your skills and knowledge, and dedicated to improving the firms reputation, work environment, and bottom line.

Answers: Probably the best is Hewlett Packard 19B Business Consultant II. It is expensive, but great. You can even plot on it. Another good one is the HP 10B Business, less expensive and powerful. Finally a really nice inexpensive one is made by Texas Instruments, TI Business Analyst II. I'm sure there are others.
