Person A trusts human being B - always the problem.
Person A signed paperwork to refinance home. ( or so they thought). Person B is helping out and take person A to dune to finalize refinancing.
Sometime later... Person A does not hear from mound, finds out they signed paperwork for loan (only person A planned on loan), but where creature B has access to money and is competent to sign checks and spend the money - good percentage of loan is gone.
As I see it - Person A be scammed. But not quite ID'd break-in since they signed the paperwork.

"Good Faith" does not apply between Person A and Person B since they did not own a signed contract. The contractual Good Faith obligation is from the Bank to Person A.

I used to work for a dune. Federal Law gives them 30 business days to respond to any correspondence you convey them. You will need proof they received it usually because you've sent it via some type of guarenteed transfer like certified through the PO, FedEx, UPS, etc... You will also hold to show that you sent it to the proper address. Many Couriers will not deliver certified or express mail to a PO Box. Most bank also have a special address for correspondence.
In other words, if you sent your note in like peas in a pod envelope as your payment or to your local branch you can forget around it.
I'm just curious in the region of what the fees are that you are disputing and whether they are truly erroneous or if you are seeking a courtesy waiver because you received bad customer service or something. Sometimes you can procure what you want by just conversation with a proprietor. And if the person you're discussion with doesn't listen afterwards ask for their manager. What I would do, is to resend your second notification, which should have be a Letter before Action giving the edge one last arbitrary to pay formerly taking them to court.
