Actually, I see how this could be confusing. Depending on how the contract is written, usually both party have to sign the agreement. It sounds similar to, based on what you wrote, that you're contracting next to a vendor, supplier, buyer, etc, but that you're using THEIR form to do it.
What you are looking at is call the "signature block" for a contract. The attestation line is for the other celebration to fill out (their heading and title) ... and I would ask them to sign it, too, right after the ":" next to "Attest".
On your side, "By:" is for your signature. "Name:" is for your printed full term to match your signature. "Title:" is for your title next to the company (are you the president, owner, director, sales head, etc?).
