You need permits if you substantially change anything. Adding a bathroom, rerouting plumbing, switching from an 80% furnace to a 92% furnace, adding central air, reroofing if you do a tear-off (but not if you go over with another layer of singles). Adding any accessory buildings, building a fence on any property line. Of course it all varies by locality.

Engineer is pretty close to correct, although some places actually require a permit for exterior paint as well, even what you can and can not plant in your yard.
It really depends on your city and your HOA.
USUALLY cosmetics (paint) are OK, but anything that requires altering the structure in any way requires a permit.
