Why did I receive two 1009 forms??

I moved a 403b to a ROLLOVER IRA then to a ROTH IRA. My considerate is that I'd only hold to pay taxes one time previously it becomes a ROTH. But beside two 1099 forms (one from the 403b, one from the ROLLOVER), it seems resembling I'm being tax for double the actual funds! When I omit one of the forms (for conducting tests purposes only), my return goes channel up! I don't understand.
