Sears merged with Kmart and ADP does their payroll. You can nickname 1-8OO-829-1040 if you do not receive your W-2 by 2-15-08 and the IRS will send them a memorandum advising K-Mart/Sears to provide you near a W2 or they will be fined by the IRS. I don't think a big company similar to Sears will wait that long. They are not required to provide a W2 until January 31st and your W2 may freshly be in the post. You can also look at your last payroll statement and you can use the year to date amounts if you own to submit a substitute W2 on Form 4852. Go to www.irs.gov and you can download any form. How do you amount out what to put into it? Go back to your payment stubs. If you have a year-end stub that have all the year-to-date income and withholding, simply take the year-end amounts and plug them into the appropriate lines on Form 4137.
